Our assessment: Silver · 75%

Three treatment rooms beside a three Michelin starred kitchen, on a clifftop over the Bay of Biscay.

The one thing

Akelarre is built around a kitchen before it is built around a spa, and the two meet in a single, deliberate gesture. The couples treatment here ends with an infusion made by chef Pedro Subijana himself, so a ritual that began with warm oil and stone finishes with something from the same three starred kitchen that has anchored this clifftop for decades. Few spas anywhere can draw that direct a line between the treatment room and the restaurant beside it.

The verdict

This is a small, honest spa attached to a serious hotel, and the honesty is worth stating plainly before anything else. There are three treatment cabins, one of them built for two, set within a wider 800 square metre floor that also holds the pool, sauna, hammam and changing rooms, so this is nothing like the sprawling resort spas elsewhere on this list. What Akelarre has instead is a position few properties can match. The hotel sits on the western flank of Monte Igueldo, looking straight out over the Bay of Biscay, and that view reaches into the spa itself, with the hydrotherapy pool and the relaxation area both facing the sea. The hotel holds Two MICHELIN Keys and the restaurant carries three Michelin stars, independently judged distinctions that place Akelarre among a small handful of properties in the Basque Country with that kind of recognition. For anyone drawn here by the restaurant as much as the view, the spa is a genuinely considered extension of both rather than an afterthought bolted on beside them.

The water

The wet area centres on a stone built hydrotherapy pool with hydromassage jets and a back waterfall, heated and facing the sea through the room's windows. A dry sauna and a marble hammam sit alongside it, and the relaxation area beyond carries the same outlook, furnished with loungers and a choice of teas and infusions for the pause between stages. Five individual changing rooms, each with its own shower and WC, keep the whole experience private rather than communal. It is a short, clean circuit built for an hour or two rather than a half day in robes, and anyone wanting a longer thermal journey with multiple heat experiences will find this a smaller stop than that.

Treatments

The menu leans on the Bay of Biscay itself. A hot stone treatment combines volcanic stones with stones gathered from the bay, and a sea themed ritual pairs Dead Sea mud and salt exfoliation with a seashell massage and a pearl and yoghurt facial, so the sea is worked into the treatments rather than just visible through the window. The signature couples ritual combines a full body massage with a cranio facial treatment and an hour in the hydrotherapy pool, sauna and steam, and closes with that infusion from chef Subijana's kitchen. With only three cabins, one of them a couples room, availability is genuinely limited, so this rewards booking ahead rather than arriving and hoping.

Getting in

The spa can be booked as a standalone experience through Akelarre's own voucher store, without needing to book a room first, which makes this one of the more accessible small hotel spas on this list for a day visit. It opens daily from ten in the morning until eight in the evening, and the fitness room beside it runs around the clock for hotel guests. San Sebastián's own airport at Hondarribia is the closest, around thirty kilometres away, with Bilbao a little over one hundred kilometres further but often better served by flights. The address sits on Paseo Padre Orkolaga on the western side of Monte Igueldo, a short drive rather than a walk from the city centre.

What to know before you fall in love

The spa is reserved for guests aged eighteen and over, while the wider hotel welcomes guests from thirteen. Winter closure dates for the spa aren't separately stated, though the property has closed over previous winters, so anyone planning a trip for December through February should confirm current dates directly before booking. With only three treatment rooms, this is a small spa by design, well suited to a focused couple of hours rather than a full day of thermal bathing.

Best for

This suits a couple celebrating a meal at the restaurant who want a treatment to go with it, and anyone who would rather have one genuinely sea facing hydrotherapy pool than a long circuit of generic heat rooms. It is a better fit for a focused few hours than a full spa day, and a weaker one for anyone who wants scale, a large treatment menu, or certainty that every fixture will be in perfect order on the day they visit.
