The one thing

Buchinger Wilhelmi Marbella is not a spa in the usual sense at all. It is a residential medical clinic built entirely around therapeutic fasting, the method its founder developed a century ago, and every part of a stay, from the morning blood tests to the four day return to food at the end, is organised by doctors around that single idea.

The verdict

Founded in 1973 by Maria Buchinger on a hillside between the Sierra Blanca mountains and the Mediterranean, the Marbella clinic carries forward a method her father Otto Buchinger first developed in Germany in the 1920s, and the family still runs both the Marbella site and its sister clinic at Lake Constance. The clinic describes itself as the world's largest centre dedicated to the science of therapeutic fasting, a claim that is the company's own rather than independently measured, though the scale of the operation, with around two hundred staff including twelve doctors, is not in doubt.

A stay here is structured rather than indulgent. Guests arrive to blood and urine tests and a consultation that sets the plan for the days ahead, and a programme typically runs from compact ten night stays to the full Classic course of three weeks or more, each one ending with several days of careful reintroduction to food. Anyone who does not want to fast at all can instead follow a plant forward, lacto ovo vegetarian eating plan, so the clinic is not a one way door.

The setting does real work here. The main hacienda and its outbuildings sit among mature palms with sea views toward the Rock of Gibraltar, and days are built around a shuttle to the beach for a sunrise walk, movement classes, and consultations with a nutritionist, a psychologist and a personal trainer alongside the medical team. It is a serious undertaking rather than a weekend away, and it suits people who want structure and are prepared to commit several days or weeks to it.

In 2026 the clinic opened Villa Mariposa, a private villa on site that was once Maria Buchinger's own home, now offered as a more intimate booking with its own pool, garden sauna and treatment rooms, and unlike the main clinic it welcomes children and dogs.

The water

There is no pool or thermal circuit at the main clinic, and that absence is deliberate rather than a gap. The clinic's whole proposition is the fast itself, days built around rest, broth, juice and guided walks rather than water. Villa Mariposa, the newer private villa on site, does have its own garden sauna and pool, so guests booking that specific villa get a small thermal element the main clinic does not offer.

Treatments

The therapy roster sits around the fast rather than replacing it, and a daily voucher is included in every stay to put toward treatments chosen with a doctor. Guests have described hydrotherapy massage, acupuncture, lymphatic drainage and osteopathy among the options, alongside yoga, Pilates, breathwork and water based classes. Those who are not fasting still see the nutritionist and the psychologist as part of the plan, since the clinic treats mind and body as one project rather than two.

Getting in

Buchinger Wilhelmi Marbella is a residential clinic rather than a drop in spa, so the way in is an enquiry through the clinic's own site, after which a programme and room are matched to the guest. Programmes run from a compact ten nights up to the full Classic course of three weeks or more, and a shorter seven night stay without fasting is also available for anyone who wants the setting and the consultations without the fast itself. The main clinic does not take children, though Villa Mariposa, its private villa, welcomes them along with dogs. Málaga airport is around an hour away by road, and the clinic can arrange a transfer.

What to know before you fall in love

  • This is a medical fasting clinic rather than a spa, so most stays involve no food at all for several days, with broth and juice in its place.
  • Programmes are residential, starting at seven nights for the non fasting option and ten nights for a fasting stay, so this suits a real commitment rather than a short break.
  • There is no pool, sauna or thermal circuit at the main clinic, only at the newer private villa, Villa Mariposa, so anyone wanting water as part of the stay should ask about that villa specifically.
  • Children are not part of the main clinic's programme, though Villa Mariposa, the clinic's private villa, welcomes them along with dogs.

We do not visit spas, so the write up above is built from published information. What it cannot tell you is how a place actually felt on the day. That part comes from you.
